In order to offer this type of care, we are stepping out of insurance and embracing a fee-for-service practice model. Direct pay is a growing healthcare model, which allows us to make your care and the time we spend with you our priority, rather than be dictated by insurance rules and mandates. We do not participate with any insurance or government healthcare programs, including Medicare and Medicaid. Full payment for your care is due at the time of service. Upon request, we are happy to provide you with a courtesy bill that you can submit to your insurance company for potential reimbursement. Reimbursement is at the sole discretion of your insurance provider. All questions regarding your insurance coverage and reimbursement should be directed to your insurance company or benefits manager.

Please note that all first time appointments in this practice will be considered an Initial Visit, even if you have seen Dr. Klemperer previously in another office. It is of the utmost importance that we establish your care in this practice. Subsequent visit types will be determined by Dr. Klemperer.
Dr. Klemperer does not have access to your prior medical records, even from her previous office. It is your responsibility to obtain those records. Please contact that office and request your records to be mailed or faxed to our office, ensuring that we receive them at least 10 days prior to your initial appointment.
Missed appointments and appointments cancelled within 48 hours of the scheduled appointment time will have a $100 fee charged.
FORMS
We ask that you complete your Intake Forms at least 10 days prior to your appointment. This will allow Dr. Klemperer to thoroughly review your history prior to your visit.
